50% deposit required at time of booking.
- Scheduled availability:
- 0700-1500: $2400, $400/additonal hr
- 0700-1700: $2800, $400/additional hr
- 0700-1900: $3300, $500/additional hr
- Endoscopy/Pain management (8 or more cases):
- Additional $500/day
- Pager call (off site, 60 minute callback):
- $1500 each 12 hours
- Callback: $300/hr, 2 hour minimum each call back
- In house call 1900-0700:
- $3500
- Holidays:
- $1000 additional, daily
- New Year’s Eve, New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, day after Thanksgiving (black friday), Christmas Eve, Christmas Day or the observed holiday.
- Travel
- $200/hr, 15 minute increments.
- Reimbursements:
- Lodging: 3 star hotel.
- Airfare: economy plus ticket.
- Vehicle: $1 / mile.
- Parking.
